Fees & Insurance

We choose to stay out-of-network so we can focus on your care, not on insurance rules. Insurance companies can control how long and what kind of treatment you get and ask for details you might not want to share. Staying out-of-network lets us put your needs first in your treatment.

Insurance

Therapists at Four Corners Counseling & Well-Being are out-of-network for major insurers, including Aetna, BlueCross, Cigna, Carefirst, UHC, and do not accept Medicare or Medicaid.

Clients pay in full at time of service via secure credit card. A Statement for Reimbursement ("Superbill") is provided for insurance claims. Many clients have out-of-network benefits covering 50% or more. Please check with your insurer, as benefits vary. Contact your Insurance Member Services to confirm coverage. You may want to ask:

  • Does my plan include out-of-network benefits?

  • If yes, what is the out-of-network deductible?

  • Are individual psychotherapy services [CPT codes 90791 (Intake session), 90834 (50-minute session) or 90837 (60-minute session)] covered by my out-of-network benefits? For relationship therapy, is CPT code 90847 covered?

  • If yes, what percentage of the Allowed Amount is covered once my deductible is met?

  • Where do I send claims for reimbursement?

Cancellation Policy

Please cancel at least 48 hours before your appointment. If you cancel late or don’t show up, you must pay the full fee. If you cancel late but book another session the same week, the fee might be waived. During bad weather, telehealth sessions are available so you don’t need to miss your appointment.
